Application Form

We offer a limited number of spots to non-students for tutoring and participation in several other enrichment programs at the Academy. Available programs include tutoring in Math and English. Tutoring and enrichment programs are offered at various times, beginning at 3:30 pm weekdays. Additional programs will be added later, and the programs offered are subject to change. Pricing is $50 per session for Private, and $40 per session for small group. Interested parents should complete the application below and contact the Academy to schedule a visit with the administration.

    Please share any other relevant detail about your child (e.g., learning abilities/disabilities, discipline issues, standardize test scores, IEP, school grades, etc.). ​Note that the Academy is not currently equipped to serve children with serious learning disabilities.
    School's Conduct Policy
    Each student is expected to comply with the Academy's conduct policy, which requires students be respectful of teachers and others from the Academy at all times. 
    Parent certification
    I/We are interested in enrolling my/our child in one or more of the above selected programs at 4Points Academy. I/We understand that a separate tuition/fee agreement and conduct policy will need to be completed. Payment for the selected program is due prior to the start of my child's participation and then at the beginning of each successive month/period of continued enrollment.

    Please be prepared to provide copies of transcripts, reports, or test scores from your child’s previous/current academic institutions (2 years).

Please drop off a check for the Application fee at the Academy or mail check with Application to our mailing address. 
Check should be made out to 4Points Academy and must be received for the application to be processed.
